[ISHM] ISHM Membership Registration update

Dear Map Historian,

The online ISHM Membership Registration Form has been amended and now it is 
already possible to enter all the required and optional membership 
information into the form (User Account) and send it to the membership 
database. 

Except for your user name you can change any information as soon as you will 
get your password. Please, do not forget to click the last two boxes and 
than the Create new account link.
